Obviously, it is polite to accede to people's preferences. Preference is best ascertained by simply asking, though clothing can sometimes be used as a rough indication -- a person wearing a dress will probably prefer to be called 'she,' though there are certainly exceptions. Guessing wrong is a trivial faux pas that should be fully atoned for by a quick apology. However, referring to a person by a pronoun against that person's stated wish is boorish.
